Social Worker Care Manager II – (250000JZ)

Position Overview

This position requires employees to work in the field (D.C.) four (4) days a week. Applicants must be authorized to work in the United States, with visa sponsorship available for qualified candidates.

Under the guidance of the Senior Care Manager, the Care Manager (CM) is responsible for providing comprehensive care coordination services to members enrolled in the Health Services for Children with Special Needs (HSCSN) health plan. The CM will regularly communicate with assigned members, assess their needs, and develop and monitor individualized treatment plans. Additionally, the role involves assisting with coordinating care and referring members to appropriate services, especially for those with complex medical and psychosocial needs.

Key Responsibilities

Case Management:

  • Manage a caseload as directed by Care Management Leadership and maintain compliance with care management requirements.
  • Conduct regular face-to-face visits with enrollees at various locations (homes, doctors’ offices, etc.).
  • Continuously assess enrollees to identify barriers and ensure they receive timely care.
  • Develop, implement, and update individualized care plans in collaboration with the enrollee’s Primary Care Physician (PCP) and multidisciplinary team members.

Care Coordination:

  • Identify and resolve any overuse or underuse of services in alignment with organizational policy.
  • Work closely with the Senior Care Manager to prioritize cases, set objectives, and report any potential risks or utilization concerns.
  • Assist with discharge planning, ensuring smooth transitions of care for enrollees.

Compliance and Documentation:

  • Ensure all care management activities are documented accurately and promptly in accordance with policy.
  • Follow all HSCSN confidentiality policies when handling sensitive enrollee information.

Educational Support:

  • Empower caregivers and enrollees by providing education on self-care, managing conditions, and understanding the importance of preventative health services.
  • Assist enrollees with appointment scheduling and track compliance for both medical and mental health services.

Multidisciplinary Collaboration:

  • Participate in professional meetings and contribute to discussions regarding treatment plans, risk management, and case prioritization.
  • Collaborate with external providers, vendors, and agencies to ensure comprehensive care.

Qualifications

Education & Experience:

  • Master’s Degree in Social Work (Required).
  • Minimum of 3 years of healthcare or related experience in managed care and/or care management (Required).
  • Minimum of 3 years of clinical or community resource setting experience (Required).

Skills & Knowledge:

  • Strong verbal and written communication skills.
  • Bilingual (Spanish-speaking) preferred.
  • Knowledge of medical terminology.
  • Proficiency in Microsoft Office and other computer systems.
  • Ability to work under pressure while maintaining a positive, customer-service-oriented attitude.

Certifications & Licenses:

  • Licensed in DC as a Licensed Independent Social Worker (LICSW) (Required).

Job Functions

Work with diverse populations: Manage complex cases, ensuring enrollees receive the necessary services and care in a timely manner.

  • Individualized Care Planning: Develop and execute care plans based on assessments of health, mental health, social, and environmental factors.
  • Collaboration with Teams: Work alongside the PCP and multi-disciplinary teams, including public agencies, to ensure the best outcomes for enrollees.
  • Monitoring and Reporting: Regularly monitor progress, report risks, and make timely interventions as needed.

Work Environment

Location:

Washington, DC – HSCSN, 3400 Martin Luther King Jr. Ave SE.

This position operates primarily in an office environment, with frequent local travel required.

Physical Requirements:

Sedentary work, with occasional lifting (up to 10 lbs), and a need for walking/standing when interacting with members in various settings.

Salary & Benefits

Salary Range:

$69,659.20 – $116,084.80 annually.

Shift & Schedule:

Full-time position with a 40-hour workweek.
