Role Overview
The Ambulatory Nurse Coordinator will join a multidisciplinary team and work under the supervision of the Division Head or Medical Director. The role involves coordinating clinical care, patient education, research initiatives, and community outreach programs. The nurse will collaborate with families, physicians, nurses, and community organizations to ensure the delivery of safe, effective, and high-quality care. This position operates on four nine-hour shifts (Monday to Friday) with a variable start time and one scheduled day off each week.
Key Responsibilities
- Coordinate all program activities related to clinical operations, education, and research.
- Conduct patient assessments, set care goals, and develop plans to improve outcomes and satisfaction.
- Participate in the creation and evaluation of division-specific policies and procedures.
- Develop and maintain databases for tracking clinical performance metrics.
- Act as liaison between the hospital and community-based healthcare programs.
- Manage smooth transitions between inpatient and outpatient care.
- Provide education to patients, families, and healthcare staff regarding medical conditions and available resources.
- Document communications, care coordination, and patient progress in medical records.
- Support and implement quality improvement and research projects.
- Maintain up-to-date knowledge of pediatric nursing best practices through ongoing professional development.
Qualifications and Skills
- Bachelor of Science in Nursing (BSN) from an accredited institution; a master’s degree is preferred.
- Current Illinois Registered Nurse (RN) license.
- At least three years of pediatric nursing experience.
- Strong communication, organizational, and interpersonal skills.
- Experience in program development and coordination.
- Proficiency with Microsoft Office applications and Epic systems.
- Basic Life Support (BLS) certification required.

Compensation and Benefits
- Salary Range: $83,200 – $137,280 annually, depending on experience and internal equity.
