What You’ll Do (Key Responsibilities):
- Personalized Care Planning: Develop and implement individualized, goal-oriented care plans to maximize functional outcomes for elderly clients, supporting them to live independently.
- Targeted Intervention: Deliver evidence-based interventions, including comprehensive falls prevention strategies, tailored education, and strength training programs.
- Holistic Assessment: Conduct thorough in-home assessments to identify physical needs, environmental challenges, and opportunities for adaptation.
- Collaborative Approach: Work closely with local community teams, families, and key stakeholders to ensure integrated and holistic client support.
- Promote Wellbeing: Incorporate a holistic approach to healthcare, integrating healthy living activities in line with community and organizational priorities.
- Quality & Documentation: Provide high-quality treatment and therapeutic interventions, maintaining accurate and detailed client records.
What You’ll Bring (Qualifications & Experience):
Essential:
- Current registration as an Occupational Therapist with AHPRA.
- Proven ability to provide high-quality treatment and therapeutic intervention.
- A valid NSW Driver’s License and access to your own reliable vehicle.
- A current Police Check (dated within the last 3 years) or willingness to obtain one.
- Evidence of up-to-date Covid-19 vaccinations.
- Willingness to obtain annual influenza vaccinations.
Highly Desirable:
Prior experience working in a community-based or home care setting.
